How to make a complaint:

If you are unhappy with the care or treatment you have received, you should first contact the person, service or organisation that provided you or your family member with that care or treatment.

If you feel you are not able to do this, or that you would prefer not to, then as the organisation that buys health services we can either help you to raise your concerns, or in certain circumstances investigate your complaint.  If it is not something the CCG can help with, someone will help to direct you to the right place.

Freedom of Information (FOI) requests

The Freedom of Information (FOI) Act was passed in November 2000. Under the terms of the Act, any member of the public can ask for access to information held by public sector organisations, including health organisations, such as clinical commissioning groups or NHS providers.  Organisations such as central and local government, the police, schools and colleges must also respond to Freedom of Information requests.

On 1 January 2005 the Freedom of Information Act was fully implemented within the NHS, allowing a general right of access by members of the public to all kinds of information held by the CCGs.  The CCGs are expected to meet any requests for information within 20 working days.

Arriving from the north via the M6

  • Leave the M6 from Junction 29 and turn right at roundabout
  • Continue over the traffic lights at Sainsbury’s
  • At the roundabout with the traffic lights, go straight over onto the A582
  • At the next roundabout go left towards Leyland
  • Continue to the traffic lights and then turn right into Centurion Way
  • Continue straight down and bear right at the roundabout
  • Take the first right turning and Chorley House in on the left approximately. 300 metres

Arriving from the south via the M6

  • Leave the M6 from Junction 29 and follow the signs for M65, Leyland, Preston South
  • Continue to the end of the M65 slip road where you will reach a roundabout
  • At the roundabout bear left onto the A582
  • At the next roundabout go left towards Leyland
  • Continue to the traffic lights and then turn right into Centurion Way
  • Continue straight down and bear right at the roundabout
  • Take the first right turning and Chorley House in on the left approximately. 300 metres

Arriving from the north or the south via the M61

  • Leave the M61 at Junction 9 on to the M65, Leyland Preston South
  • Continue to the end of the M65 slip road where you will reach a roundabout
  • At the roundabout bear left onto the A582
  • At the next roundabout go left towards Leyland
  • Continue to the traffic lights and then turn right into Centurion Way
  • Continue straight down and bear right at the roundabout
  • Take the first right turning and Chorley House in on the left approximately. 300 metres

Getting here if you are travelling by public transport

By Bus from Leyland Railway Station

Leaving Leyland Railway Station, cross over the road, turn left, continue down hill to the bus stop.  Get the 111 or 113 bus, ask the driver which stop you will need for Centurion Way.

Walk down to the end of Centurion Way and turn right on to Lancashire Business Park past the Gatehouse, and then take the first turning on the right.  Chorley House is on the left after approximately 300 metres.

On foot from Leyland Railway Station (approximately 30 minutes walk)

From Leyland Railway Station, cross over the road, turn right and walk over the railway bridge.  Go straight ahead across a small roundabout, passing the Railway Public House on your left and a car dealership on your right.  Continue along the same road, passing shops, houses, a park and Farington Lodge Hotel, until you come to a set of traffic lights.  Turn left at the lights on to Centurion Way, Walk down to the end of Centurion Way and turn right on to Lancashire Business Park past the Gatehouse, then take the first turning on the right.  Chorley House is on the left after approximately 300 metres.
